The depression, anxiety, bipolar disorder, and ADHD screening questionnaires are scored locally in your browser. They do not request your name or contact information, transmit your selected answers to Edwards Psychiatric Care, or save your answers after you leave or reload the page.

The questionnaire pages and responses are not monitored. Screening results are educational and are not part of a medical record unless you independently choose to discuss them during a clinical appointment.
